
What to Expect from Your Evaluation
At OneRehab, we understand that seeking a neuropsychological or psychological evaluation can feel overwhelming. Our goal is to make the process seamless, informative, and tailored to your specific concerns. Here’s what you can expect from start to finish.
Evaluation Day
At OneRehab, we strive to provide a thorough and supportive evaluation experience. Here’s what you can expect:

We will discuss your concerns, medical history, and reasons for the evaluation to ensure a tailored assessment.

You will complete a series of standardized tests that assess cognitive functions such as memory, attention, problem-solving, and language.

Evaluations typically last 3-4 hours, depending on the complexity of the case. Breaks are provided as needed.

A follow-up appointment will be scheduled to review your results, discuss findings, and provide recommendations for treatment or accommodations.
What to Bring to Your Appointment
To ensure a smooth and efficient evaluation, please bring the following items to your appointment:

A valid government-issued ID (e.g., driver’s license, passport).

If applicable, bring your insurance card for verification.

Any relevant medical, neurological, or psychiatric records.
Previous Testing Reports – Copies of prior neuropsychological, psychological, or educational assessments.

A current list of all medications, including dosages.
Eyeglasses and Hearing Aids – If you require them for reading, writing, or hearing.
Comfortable Clothing – Dress in layers to ensure comfort during testing (e.g., sweater in case it becomes cold).

You may bring light snacks and drinks. We will also be providing water, tea, coffee, etc.

You should receive a link to fill out your patient intake forms in your initial email. If you haven’t or if you prefer to fill them out and bring it in with you, please use the link attached to access them. We recommend you sending in your intake forms via email/fax to our front office team 24-48 hours prior to your appointment.
